1. Is the city attorney available for personal use/counsel?

No, unless there is a legal issue regarding you as a city employee or your department.

2. How do I know if I'm eligible for Family Medical Leave (FML)?

There is a section in the Employee Policy Manual devoted to FMLA. Please review that section. As always, the Human Resources Department is here to help you. Our number is 501.982.4671.

3. If I move, what do I need to do?

Once you have moved, there are certain steps that must be taken to ensure that the City and your benefit providers know how to get in contact with you. 

Employees must notify their supervisors of this change AND human resources.

Police or Fire Department employees must see the Chief's admin and request the "Change of Address Packet". Once that is completed, turn that back into the Admin and they will submit the packet to Human Resources. 

If you have moved and are no longer employed with the City, we'll need the corrected address for W-2's.  We will need that change in writing.

4. When can I make changes to my benefits?

Unless it's during an open enrollment period, you must have a qualifying event. A qualifying event is defined as a marriage, divorce, birth, death, or adoption. Your changes must be made within 30 days of the qualifying event.

If proper changes are not made within 30 days, you will have to wait until the next open enrollment period.

5. If I get called for jury duty, am I required to give the city my "Jury Duty Check?"

Yes, but only if you have jury duty on a day that you would otherwise be at work.  You will be paid your regular rate of pay by the City for time served on Jury Duty. Therefore, you must sign your "Jury Duty Check" over to the City, so you are not paid twice.

6. I received a check from the Arkansas Municipal League (ARML)for my work related injury. What do I do with it?

If you are off work and receive a workers compensation check from ARML, it must be signed over to the City of Jacksonville and turned in to the Human Resources Department. The City compensates you 100% of your pay while you are out with your work related injury. The check from ARML reimburses the City.

7. If I am hurt while at work and choose not to seek medical attention immediately, what happens if I later need to go to the doctor?

Regardless of whether or not you choose to go to the doctor, your Supervisor must be informed of the incident immediately. If you feel that you do not need a doctor immediately, that is fine. If, down the road, you feel that you should probably go see a doctor because there is still an issue, notify your Supervisor and then Human Resources. HR will take care of making you an appointment.

After leaving the doctor's office, you must visit Human Resources to complete the proper Worker's Compensation paperwork.
